Westside Gathering (WSG) is collecting & retaining this personal information for the purpose of enrolling your child(ren) in our programs, to assign them to the appropriate group, to develop & nurture ongoing relationships with you & your child(ren), and to inform you of program updates & upcoming opportunities at our church. This information will be maintained permanently in our online database as it is a requirement of our insurance company & legal counsel.

I/We the parent(s) or guardian(s) named below, authorize the ministry staff of WSG to sign a consent for medical treatment and to authorize any physician or hospital to provide medical assessment, treatment or procedures for the participant(s) named above. I/We, named below, undertake and agree to indemnify and hold blameless the ministry staff and WSG from and against any loss, damage or injury suffered by the participant as a result of being part of the activities of WSG KidsQuest, as well as of any medical treatment authorized by the supervising individuals representing the church. This consent and authorization is effective only when participating in WSG events.

Photo/Video Release *

By selecting Yes below, I give WSG permission to take photos/videos of my child/children and use their picture solely on the church's website, social media (Facebook/Instagram) and at/to publicize KidsQuest or WSG events. WSG will never publish a child's name with any of its publications. Should you select No and disagree, we will do our best to ensure that your child does not appear in photos/videos*.

*If you allow your child(ren) to participate in special performances on stage during the gathering, they WILL appear on the livestream and pictures/videos may be taken by people in the audience (nametags should be worn on their backs for these performances).
